Dr. Judy Fernando Graduated from Monash University in Melbourne in 2000. She spent first 3 years of her medical including her intern year at Monash Medical Centre. She did her GP training in Melbourne and received her Fellowship in the Australian Royal College of General Practitioners in 2006.
She worked as a GP in Melbourne before she decided to move to sunny QLD. In 2009 She worked as a solo practitioner at Middlemount, a rural mining town where she was exposed to the challenges of industrial medicine. Since then she perused her career expansion in occupational medicine by gaining Post Graduate Diploma in Occupational Medicine. She is an Approved Health Practitioners (AHP) for Health assessment for rail safety workers and Nominated Medical Adviser (NMA) for Coal Board medicals.
Dr. Fernando became the director of Hamilton Medical Centre and Brisbane Industrial Health services in February 2015 after Dr. Greg Cunningham’s retired. She is a local resident and is married with 2 children.

Dr Tiffany Flynn: MBBS (Hons) BMedSc, FRACGP, FARGP, Dip Child Health ,DRANZCOG Adv.
Dr Tiffany Flynn graduated from Griffith University in 2011 after obtaining a Bachelor of Medicine and Surgery. She spent her years as a junior doctor at Toowoomba Hospital before moving to Bundaberg to complete an advanced Diploma in Obstetrics and a Diploma in Child Health. She worked as a GP obstetrician for 12 months in Chinchilla before moving back to Toowoomba where she obtained her Fellowship in General Practice and Rural General Practice. Dr Flynn is also working in medical education with the University of Queensland. In her down time she enjoys spending time with her husband, skiing and training her border collie.

Hamilton Medical Centre is a Family Medical Practice that was established in1986 and is running under new management since 2nd of February 2015 .The doctors and staff at Hamilton Medical Centre are committed to providing the highest standard of medical care in a friendly and courteous setting. We are a mix billing practice.

Standard consultation A$ 70 ( Medicare rebate applies ) Monday to Friday 8am to 5 pm.
Bulk billing is available for children under 16 years of age, Health Care Card holders and Pensioners Monday to Friday 8am to 5 pm. Saturdays and after 5 pm Monday to Friday cost of standard consultation is $75 and $60 for children under 16 years of age, Health Care Card holders and Pensioners . Please note procedures will not be bulk billed.
Full fee structure is available at the reception, varies for different procedures. We are able to process Medicare claims and can automatically arrange for the Medicare rebate to be paid to you on the day.
This practice accepts payment of accounts by Cash, Cheque, EFTPOS or Credit Card.

The practice provides 24 hrs patient care from our doctors and our Locum service. Please ring 137425, if you need to contact a doctor after hours. Please note you need to call 000 if it is a medical emergency. Home visits if medically needed are available by prior arrangement with your doctor if you live close proximity to our surgery.

Appointments
The easiest way for you to see your doctor is to make an appointment. Doctors make every effort to see appointments as close to time as possible, although unavoidable delays can occur when our doctors must attend to a patient with complex or serious medical conditions. Some consultations may require a longer appointment for things such as insurance/pre-employment medicals or you may have problems that need more time to discuss. Please inform the receptionist when booking if you may require an extended appointment. We encourage you to see the doctor of your choice to ensure continuity of care and the establishment of a professional relationship.

Cancellations
If you are unable to attend your appointment please notify us of this in plenty of time, so your appointment time can be allocated to someone else. Please be advised that repeated non-attendance without cancelling appointments will incur a fee, which is non-refundable by Medicare.

Test Results
All test results are checked by your doctor. Most results are back with your doctor within 72 hours. Unless otherwise advised all patients are asked to make an appointment to discuss results with their doctor. This ensures your confidentiality and the appropriate action for your result. A normal result may not mean that no further action is necessary.

Reminder System
Our practice is committed to preventative care. We may issue you with a reminder notice/call offering you preventative health services appropriate to your care.

Emergencies
Priority treatment is given to seriously ill or injured patients. We are able to deal with most emergencies and trauma. Please notify the receptionist on arrival.

Surgical Procedures
To ensure the health and safety of our patients, all instruments used in procedures are sterile, or sterile disposable instruments are used . All surgical procedures incur gap fee. Charges may vary depending on the procedure.

Health Promotion and Vaccination
We offer comprehensive health assessments, Care plans, diabetes education , smoking cessation, weight management and antenatal shared care. The full complement of childhood and travel vaccinations are available (including Yellow fever and Q Fever). Please ask the receptionist for further information.

Repeat Prescriptions
Repeat prescriptions are only issued for existing conditions at the doctor’s discretion. A fee is charged for all scripts issued without a consultation appointment.

Referrals and Medical Certificates
An appointment with your normal general practitioner is usually required to receive a referral to a specialist. For referral extensions, a fee is charged where a referral is written without a consultation appointment. Medical Certificates always require a consultation.

X-rays and Scans
We can only keep your X-rays, scans , Discs or films on the premises for a period of three months and then will have to dispose of them. Please collect your diagnostic images from the practice.

It is the policy of the practice to maintain security of personal health information at all times to ensure that this information is only available to authorized members of staff.
